Calidad de vida del paciente a los seis meses del alta de una unidad de cuidados intensivos

    Objetivo: evaluar la calidad de vida relacionada con la salud (CVRS), de los supervivientes de una enfermedad crítica a los seis meses del alta de una unidad de cuidados intensivos (UCI). Método: estudio observacional de carácter longitudinal realizado en la UCI de un hospital universitario de nivel terciario. Población: todos los pacientes adultos ingresados en UCI, mayores de 18 años, con una estancia > de 48 horas, durante el período del 18 de septiembre del 2020 al 18 de abril del 2021. Criterios de exclusión, pacientes con dificultad con el idioma y personas con alteraciones neurológicas que impidan su comunicación o psiquiátricas graves. Durante las primeras 48 horas del ingreso del paciente en UCI, se le realizó una encuesta de calidad de vida (CV), con el cuestionario EuroQol (EQ-5D-5L), donde se le preguntó por su CV durante los cuatro meses anteriores a su ingreso en UCI. A los 6 meses del alta, se contactó con los pacientes, vía telefónica, para realizar de nuevo la encuesta de CVRS. Resultados: se incluyeron en el estudio 125 pacientes que cumplían los criterios de inclusión, 27 pacientes fallecieron durante el ingreso en UCI y 12 pacientes EN los 6 meses después del alta. El total de pacientes que completaron el cuestionario a los seis meses fue de 86. Los resultados del cuestionario de salud (EQ-5D-5L) a los seis meses del alta, presentan un aumento de los problemas en las dimensiones de la movilidad del 34,9%, en el auto cuidado el 19,8%, en las actividades cotidianas el 40,7%, en el dolor/malestar el 22,1% y el 26,8 % más de ansiedad/depresión, de la que tenían antes de la enfermedad. Conclusiones: en nuestra investigación hemos observado una disminución de la calidad de vida percibida por los pacientes a los seis meses del alta de UCI.

    Objective: Evaluate the health-related quality of life (HRQOL), of the survivors of a critical illness six months after being discharged from an intensive care unit (ICU). Method: longitudinal observational study carried out at the ICU of a tertiary university hospital. Population: all adult patients admitted into the ICU, older than 18, staying there for > 48 hours, during the period between the 18th of September, 2020 and the 18th of April, 2021. Exclusion criteria, patients with language barriers and people with neurological disorders that affect their communication skills or people with severe psychiatric disorders. A health-related quality of life survey (QOL) was conducted on the patients using the EuroQol questionnaire (EQ-5D-5L), during the first 48 hours that they were in the ICU, where they were asked about their QOL during the four months before being admitted into the ICU. 6 months after being discharged, the patients were contacted over the phone to conduct the HRQOL survey again. Results: 125 patients who fulfilled the inclusion criteria were included in the study, 27 patients died while they were in the ICU and 12 other patients died within 6 months from being discharged. So a total of 86 patients completed the questionnaire after six months from being discharged. The results of the health questionnaire (EQ-5D-5L) six months after being discharged, showed an increase in the problems related to mobility 34.9%, self-care 19.8%, daily activities 40.7%, pain/discomfort 22.1% and 26.8% more from anxiety/depression, than what they had before the illness. Conclusions: In our research we observed deterioration in the quality of life perceived by the patients six months after being discharged from the ICU.
