Historia clínica electrónica: Un instrumento para la atención farmacéutica

    • Introduction: The implementation of systems with electronic medical records is an increasing trend in the hospital environment. Our goal is to determine its usefulness as a tool for developing primary care activities directed at optimizing pharmacotherapy for the elderly population. Method: Two-stage prospective study of pharmaceutical interventions. In both stages, we identified potentially inadequate drugs for the elderly population according to the Beers criteria. After the initial observational stage, we included red flag notes in the electronic medical records of those patients with pethidine, ketorolac, two or more benzodiazepines and long half-life benzodiazepines warning about their risks and proposing safer therapeutic alternatives. Results: In the first stage, 2.18% (102/4,677) of prescriptions came from drugs included in the Beers criteria. This percentage fell to 1.0% (41/4,091) in the second stage. The prescription of potentially dangerous drugs for the elderly decreased (relative risk reduction: 54.0%; confidence interval [CI] at 95%: 34.1-67.9 and absolute risk reduction: 1.18; C195%: 0.6-1:7). Discussion: Pharmaceutical interventions based on electronic medical record systems are a potentially useful tool for increasing safety in drug use for the elderly patient
